Patient may not participate in Medicare, Medicaid, or another federally funded healthcare program. Total program benefits $5,000 per patient per 6-month-calendar period. Patient is responsible for costs above this amount. Program does not cover office visit, physician, staff, or administrative charges associated with administering XGEVA®. XGEVA® is not indicated for the prevention of skeletal-related events in patients with multiple myeloma.
